Payment is due at the time of your appointment.

All payments are processed securely through Tebra, which supports:

• Credit and debit card payments

• Automatic receipts

• Secure storage of a card on file

Tebra applies a standard credit card processing fee, which is charged by Tebra—not by my practice.

A card on file is required for scheduling and for all active patients

Although I am currently a cash-pay practice, I can provide a superbill upon request.
This document allows you to submit a claim to your insurance provider for possible out-of-network reimbursement.

Reimbursement depends entirely on your individual insurance plan benefits and is not guaranteed.

I am actively working through the credentialing process and expect to begin accepting insurance in the near future.

I am in the process of credentialing with:

• Aetna

• Anthem Blue Cross Blue Shield

• Cigna

• UnitedHealthcare / Optum

• UMR

• Colorado Medicaid (Health First Colorado)

I will not be accepting Medicare.

Once insurance enrollment is complete, patients will be able to utilize their benefits for eligible services.

Until then, all appointments will remain self-pay through Tebra.

Even after credentialing is complete, some services are typically not covered by insurance, including:

• Many functional or specialty laboratory tests

• Supplements

• Extended consultation time beyond medical necessity

• Certain integrative or lifestyle-focused assessments

I will always discuss optional and non-covered services before proceeding

Most services can be paid for using Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA).
Please check with your plan administrator to confirm eligibility.

Fees vary based on visit type, duration, and insurance coverage.
Cost estimates will be reviewed prior to scheduling and confirmed before services are provided.

These fees may be adjusted as needed and will be clearly communicated.
